Quality Improvement Plan Navigator
The Quality Improvement Plan (QIP) Navigator is Ontario Health’s secure online platform for developing, submitting and accessing annual QIPs.
The QIP Navigator gives teams a secure workspace to develop their plans, access historical QIPs, search for QIPs by keywords or indicators through Query QIPs and download previously submitted QIPs.
Quality Improvement Plans
A Quality Improvement Plan (QIP) is an organization-specific plan that outlines annual quality improvement priorities, activities and targets. QIPs:
- align QI work with system-wide quality goals and local priorities
- support organizational accountability and continuous QI
- highlight performance data, targets and improvement strategies
- promote transparency and shared learning across sectors
QIPs are submitted to Ontario Health annually by April 1 through QIP Navigator and published publicly.

Submit QIP
Submit your organization’s annual QIP through the secure online portal.

Access support materials, including:
- annual guidance documents and sector-specific technical specifications.
- user guides, checklists, QI best practices and YouTube how-to tutorials.

Download QIPs
Download historical QIP submissions for analysis, comparison, or benchmarking.
Query QIPs
Use Query QIPs to:
- search submitted QIPs by keywords or indicators
- generate reports on Workplan, Progress Report and Narrative sections
- export query results in multiple formats for analysis and planning
