The purpose of this dataset is to securely store the data collected and managed by CHCs, which provide primary care, health promotion, and community programs.
- Need for Personal Health Information (PHI)

PHI is required to:
- support the OHT Data Dashboard for OHTs and its partners, provide data for community health and primary care topics of the OHT Data Dashboard
- report equity relevant data on marginalized populations for OHTs and their partners
- develop measures of primary care patient attachment
- support identification of patients who receive regular primary care services at CHCs.
- Why De-identified and/or Aggregate Information will not Serve the Identified Purpose
- PHI-level data is required for data linkage.
