eForms: Description of Services

Ontario Health eForms is an online platform for authorized users to submit electronic forms such as the Ministry of Long-Term Care Health Assessment Form and Ministry of Transportation forms. This platform provides access to a library of digital forms through integration with electronic medical records and accessed directly through a OneID account. It allows for pre-filling of patient and provider demographic information, attaching files, confirmation of receipt for submitted forms, and tracking the status of these submissions.

Ontario Health has physical, administrative and technical safeguards in place to protect personal information (PI) and personal health information (PHI) against loss, theft, unauthorized access, disclosure, copying, use or modification.
