Position Summary

OH Toronto is seeking expressions of interest from qualified healthcare professionals from various health disciplines such as nurses, social workers and others, excluding physicians, for the position of Regional Clinical Co-Lead, Palliative Care [Regulated or Legislated Healthcare Professional, excluding Physicians]. This is a unique opportunity to collaborate with regional partners and provide clinical leadership to the evolution of the palliative care system in Ontario. This role will report directly to the Vice President, Clinical Programs and Capacity Access and Flow in Toronto Region. In addition, this role will have indirect reporting and accountability to the Provincial Clinical Co-Leads, Provincial Palliative Care Program. This position plays an instrumental role in shaping regional palliative care delivery systems and implementing regional initiatives aligned to provincial priorities.

Accountabilities

  • Provide clinical leadership and advice to support high quality, standardized, regional palliative care program development and implementation
  • Participate in horizon and jurisdictional scanning to stay informed of emerging knowledge and best practices, and to identify and understand differences in policy and practice between Ontario and other key jurisdictions
  • Build consensus among various clinical experts to drive program evolution and adoption related to provincial and regional priorities aligned to the provincial Quality Standard, Health Services Delivery Framework Recommendations and other guidelines
  • Work with regional leadership and relevant partners to identify and achieve region-specific goals and objectives
  • Work with regional partners to drive implementation, monitoring and improvement of regional activities in support of system-level provincial plans and initiatives
  • Ensure coordination and integration of care in the region by working with local partners and service delivery organizations, including Ontario Health Teams (OHTs), to align with provincial models of care recommendations as outlined in the Health Services Delivery Framework
  • Support establishment of person-centred transitions across care settings in the region by working with local partners and service delivery organizations, including Ontario Health Teams (OHTs), to align with provincial models of care recommendations as outlined in the Health Services Delivery Framework
  • Support targeted regional activities to address issues of capacity, access and flow
  • Work with regional partners to increase equitable access to community palliative care services and outcomes for vulnerable and marginalized populations and other equity-deserving groups
  • Work closely with regional palliative care providers, and other local care providers to identify and address palliative care educational needs across care settings, based on the Ontario Palliative Care Competency Framework, aiming to build capacity to support implementation of provincial models of care
  • Work with regional administrative leadership to ensure there is a strong foundation in place, including the Regional Palliative Care Network (at OH Region or sub-region level) or similar body, to support program development, implementation and performance management at the regional level
  • Represent the corresponding OH Region at key regional and provincial clinical leadership tables, and lead regional interdisciplinary collaboration to support primary level palliative care capacity building
  • Participate actively, along with other clinical and administrative leaders, in palliative care to define, guide, monitor and lead the development and delivery of coordinated standardized, high quality palliative care services
  • Work closely with partners in the region to ensure that data, reporting, and digital infrastructure support service providers with monitoring and improvement efforts

Qualifications

  • Practicing healthcare professional (e.g. nurse, social worker, or other, excluding physician) who is in good standing and belongs to a profession which falls under the Regulated Health Professions Act or is regulated by other legislation in Ontario.
  • A minimum of five years of clinical experience in palliative care
  • In-depth understanding of Ontario's health care system, with particular focus on the challenges and issues facing palliative care in Ontario
  • Expert knowledge of current best practices, healthcare literature, evidence-based guidelines and the provincial Quality Standard, as it pertains to palliative care, including integration of palliative care in the community, long term care homes, and Ontario Health Teams through implementation of the provincial Health Services Delivery Framework Recommendations
  • Knowledge of health system performance measurement and management approaches, and how this can be used as a tool to promote change 
  • Aptitude for system thinking and ability to analyse information for sound decision making
  • Excellent communication skills with mature leadership experience in establishing strong relationships and building consensus with partners
  • Demonstrated commitment to equitable and person centred care, as well as public accountability
  • Demonstrated experience in implementation of large-scale change in the health sector
  • Demonstrated ability to work with diverse stakeholders to reach consensus

About the Ontario Palliative Care Network

Established in 2016, the Ontario Palliative Care Network (OPCN) is a partnership of health service providers, community and social support service organizations, health systems planners, as well as patient and family/care partner advisors formed to develop a coordinated and standardized approach for delivering palliative care services in the province. 

The Provincial Palliative Care Program is responsible for executing OPCN’s mandate and priorities in alignment with the Ontario Health role.

Mandate

  • To act as a principal advisor to the government on quality, coordinated, palliative care in Ontario.
  • To be accountable for quality improvement initiatives, data and performance measurement, and system-level coordination of palliative care across the province.
  • To lead regional and local integration of palliative care services and delivery.
  • To engage sector interest holders including but not limited to, Francophones, and First Nations, Inuit, Metis and urban Indigenous communities.

The Regional Clinical Co-Leads play an important role in the execution of the OPCN priorities, especially leading regional initiatives related to clinical improvements in palliative care in Ontario. In addition, the Regional Clinical Co-Leads play a significant role in promoting and ensuring implementation of the Tools and Resources created by the OPCN.

Effective regional engagement and relationship building are essential to executing the mandate of the OPCN as well as OPCN priorities. The Provincial Palliative Care Program staff as well as OH Regions support and work with the Regional Clinical Co-Leads to enable them to carry out their roles.
