eForms
Developed by Ontario Health, eForms is a new initiative designed to reduce the administrative burden for health care providers in Ontario.
The primary purpose of the eForms platform is to minimize reliance on paper and fax-based workflows, eliminate redundancies caused by incomplete or incorrect information and offer a more secure and efficient way to complete forms.
Platform Features
- Direct integration with electronic medical records (EMRs) – Clinicians, administrative assistants or other authorized individuals can launch the system directly from a patient's chart in the EMR. The eForms platform also supports EMR-agnostic access via ONE Health, allowing users to securely access and submit forms.
- Pre-population of information – Digital forms are opened in a separate browser tab with pre-populated patient and provider information.
- Tracking and management – Users can view forms previously submitted via eForms under the same health information custodian's authority.
- Managing submitted forms – The eForms dashboard tracks for receipt confirmation upon submission.
- Bidirectional communication – Users can exchange messages within eForms, enabling follow-ups, requests for additional information, and status updates within the platform.
Platform Benefits
- Efficient form submission – Administrative forms are sent securely to the intended recipient via the eForms platform.
- Automatic notification – Recipients, such as Ontario Health atHome coordinators, receive notifications on newly submitted forms.
- Status updates – Changes to form status, by either clinicians or recipients, are visible through the platform for all users.
- Reliable transmission – Eliminates the inefficiencies of fax-based communication, confirmation of receipt is visible, and the digital format ensures completion of mandatory form components.
- Enhanced accessibility – Clinicians can search and auto-fill forms with patient and or provider information.
- Secure access – User access to digital forms is protected by the provincial ONE ID account.
Digitized Forms
The OH eForms platform currently includes the following priority forms as identified by the Ministry of Health and Ontario Medical Association:
- Ministry of Long-Term Care – Health Assessment Form
- Ministry of Transportation – Driver Medical Report
- Ministry of Transportation – Medical Condition Report
- Ministry of Transportation – Condition Specific Report

Future Releases
The eForms platform is being expanded in phases to include additional provincial forms. Future releases will continue to support digital transformation by improving efficiency, reducing manual work and streamlining communication between clinicians and program recipients.
Future updates may introduce features such as transmission of clinical data, multi-provider completion, form cloning, recipient-initiated forms, assigning delegates, the ability to access forms submitted by other providers for the same patient, technology upgrades, and ongoing addition of new forms.
ONE ID Registration
All users across Ontario are invited to access the eForms platform. To ensure secure access and protect patient information, users are required to have an ONE ID account. ONE ID provides a single, secure login that verifies your identity and allows you to access Ontario Health digital health services, including the eForms platform.
