Supportive Housing Services in Ontario Health West

Supportive housing development in Guelph, Ontario ensures residents with complex, high-acuity physical, mental, and substance use health needs receive supports to manage health and well-being, improving their quality of life.

On June 26, 2024, a 32-unit supportive housing development was opened at 10 Shelldale Crescent in Guelph, Ontario. This initiative aims to support individuals with complex, high-acuity physical, mental, and substance use health needs, through a supportive housing model. The housing development includes access to comprehensive, wrap-around healthcare services within the building.

10 Shelldale is a partnership between Kindle Communities, Guelph Community Health Centre and Stonehenge Therapeutic Community. Ontario Health supported local health system planning and provided oversight to this partnership so health service providers could deliver integrated healthcare services. 

This partnership is a robust community effort to integrate supportive housing, healthcare and social services. This increased access to services improves health outcomes and quality of life for the region’s most vulnerable members, including individuals experiencing chronic houselessness.

Healthcare services offered at 10 Shelldale include:

  • primary care
  • mental health counselling
  • substance use health counselling
  • recreation therapy
  • peer support
  • nutrition support

This collaborative, community-led model is designed to deliver holistic care, aimed at addressing various aspects of residents’ health and well-being.

Along with shared common and social spaces, residents have access to 24/7 on-site supports, including mental health and substance use health services. This integrated model ensures that residents receive the necessary support to manage their health and improve their overall quality of life. 10 Shelldale is located near transit, employment opportunities and other social and health services that will further contribute to positive outcomes for its residents.
