Virtual Care Information to Share With Patients

Hypertension log

Patients can track their blood pressure at home using a fillable log that collects their systolic and diastolic readings.

Pre-appointment survey

Clinicians can use this survey to collect patient reported information that may be needed before a virtual care appointment. This survey is available in a standard survey template, OceanMD template and Microsoft Forms template.
